The apostolate is entrusted to the Congregation of the Sacred Hearts of Jesus and Mary (SS.CC.). In the United States it is promoted by the National Enthronement Center of the Sacred Heart in the Home, Fairhaven, Massachusetts, with the help of Diocesan Centers, the Men of the Sacred Hearts, the Legion of Mary and lay volunteers.

Members have a portal to sign in to.
This is not a membership you hold but a single ceremony your household goes through, followed by a way of living afterwards. The family puts an image or statue of the Sacred Heart in a place of honour, a throne, and then kneels together and consecrates the home to Christ, asking him to reign over the household. Everyone signs a Certificate of Enthronement at the end, which most families frame. It takes about twenty to thirty minutes. It is traditional to invite a priest, but a priest is not required: the head of the household may lead it. It works just as well in a single-parent family or for someone living alone.
Where it comes from
Begun by Fr Mateo Crawley-Boevey, SS.CC., who started promoting home enthronements in 1907; the practice was approved by Pope St Pius X in 1908. The devotion itself goes back to the visions of St Margaret Mary Alacoque in the 1670s.
What you promise
Nothing binds under sin and there is no register. What the apostolate asks for is follow-through: observing the First Friday of each month with Mass and a renewal of the consecration before the image, praying the family Rosary in front of it, keeping the feast of the Sacred Heart each year with Mass and Communion, and renewing the Act of Consecration at anniversaries, births, baptisms, First Communions and in times of joy, illness or loss.
What the Church attaches
The promise attached to the devotion, as recorded from St Margaret Mary, is that Jesus will bless those who honour his Sacred Heart and display his image in their homes. The pages retrieved describe the Enthronement as a source of strength and blessing for the family, a recommitment to baptismal promises, and a spur to Eucharistic life and charity to neighbours. No indulgence is published on either page, so none should be claimed.
Something physical is involved
A framed image or statue of the Sacred Heart of Jesus for a permanent place of honour in the home, blessed and sprinkled with holy water during the ceremony, plus a Certificate of Enthronement signed by the family and the presider.
